New Patient Registration
If you are resident within our practice area (please see above) and would like to register with the practice, please speak to one of our receptionists who will explain the procedure and help with the completion of the registration form. You will also be asked to complete a questionnaire in order to ensure that our computer holds an accurate record of your past medical history. Both forms are available on the above “Registering Online” tab.
If you wish, you can opt to have a new patient health check with the healthcare assistant: please ask reception.
Patients need to inform the practice of any change in personal details e.g. telephone number, address or surname. Confidentiality will remain of prime concern.
Please note that the practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.
Registered patients who subsequently move out of our practice area will be required to sign up to an alternative GP surgery.
Patient Preference of Practitioner
Patients are registered with the practice, not an individual GP. For administrative reasons your medical card will be issued in the name of one of the doctors; however, you can at any time express a preference for a particular doctor, for either all of your medical needs or on a case by case basis. That said, not all the doctors in the practice provide all services and specific doctors may not be immediately available.
